Validação do protocolo de avaliação dos distúrbios adquiridos de fala em pacientes com doença de Parkinson e do Radboud Inventário Motor Oral para doença de Parkinson

Abstract: Base teórica: Manifestações motoras da doença de Parkinson (DP) podem influenciar na produção da fala, sendo a articulação um dos aspectos fonoaudiológicos mais comprometidos na doença, e de alta prevalência nesta população. A partir destas informações, justifica-se pesquisar sobre protocolos que auxiliem na avaliação clínica dos distúrbios adquiridos de fala. Além destes de protocolos, os questionários autoavaliativos são muito importantes, pois a percepção dos pacientes sob estas alterações contribui para a acurácia da avaliação fonoaudiológica, e identificam a influência destas alterações na qualidade de vida dos pacientes. São escassos os instrumentos válidos, e a validação deve ser realizada com o intuito de obter resultados mais precisos e fidedignos. Objetivo: elaborar e validar um protocolo específico para avaliação dos distúrbios adquiridos de fala em indivíduos com doença de Parkinson (PADAF), e realizar a tradução e validação linguística e cultural do questionário Radboud Oral Inventory Motor for Parkinson´s disease (ROMP). Métodos: Para elaboração e realização da validação de conteúdo e verificação da confiabilidade entre examinador do Protocolo de Avaliação dos Distúrbios Adquiridos de Fala em Indivíduos com Doença de Parkinson (PADAF), o estudo foi realizado em três etapas: Na primeira, foi elaborado o protocolo e validado seu conteúdo através da análise de sete especialistas. Na segunda, aplicou-se o instrumento em 25 pacientes com Doença de Parkinson Idiopática (DPI) Na terceira e última etapa, verificou-se a confiabilidade entre examinador. Para realizar a tradução e validação linguística e cultural do questionário “Radboud Oral Inventory Motor for Parkinson´s disease (ROMP), o instrumento foi traduzido e retraduzido, e a confiabilidade foi verificada através da análise da consistência interna e da reprodutibilidade do reteste intraexaminador, sendo a versão final aplicada em 27 participantes com doença de Parkinson (DP). Resultados: A versão final do PADAF foi composta por 32 itens que avaliam a respiração, a fonação, a ressonância, a articulação e a prosódia. Mostrou-se válido, com índice de validade de conteúdo (IVC) bem acima daquele estabelecido na literatura e com perfeita concordância na verificação da confiabilidade entre examinador. Quanto ao ROMP, a consistência interna foi de 0,99 para o ROMP total e de 0,96 a 0,99 para os 3 domínios. Os coeficientes de correlação intraclasse para reprodutibilidade foram de 0,99 para o ROMP total e de 0,93 a 0,99 para as subescalas. O ROMP e suas subescalas correlacionaram-se substancialmente com a escala do tipo Likert, bem como com os itens UPDRS II e III. Conclusão: O PADAF para indivíduos com DP foi desenvolvido e teve seu conteúdo validado com perfeita confiabilidade do instrumento, assim como houve equivalência linguística e cultural do ROMP no português brasileiro, com excelente confiabilidade e validade. Ambos instrumentos mostraram-se úteis e aplicáveis para esta população. ; Theoretical basis: motor manifestations of Parkinson's disease (PD) may influence speech production, with articulation being one of the most prevalently affected speech subsystems in this population. These findings justify research on protocols that assist in the clinical evaluation of acquired speech disorders. In addition to these protocols, self-evaluation questionnaires are very important because the perception of individuals with these impairments contributes to the accuracy of the speech-language pathology assessment. Additionally, they help qualify the influence of these speech alterations on the quality of life of patients. While valid instruments are scarce, validation must be performed in order to obtain more accurate and reliable results. Objective: to elaborate and validate a specific tool for the evaluation of acquired speech disorders in individuals with Parkinson's disease (PADAF), as well as to perform the linguistic and cultural translation and validation of the Radboud Oral Motor Inventory for Parkinson´s Disease (ROMP). Methods: for content validation and verification of reliability between examiners using the Protocol for the Assessment of Acquired Speech Disorders in Individuals with Parkinson's disease (PADAF), the study was carried out in three stages. First, the protocol was elaborated and its contents validated through the analysis of seven specialists. Second, the instrument was used to assess 25 patients with idiopathic Parkinson's disease (PDI). In the third and final stage, reliability was verified between examiners. For the translation and linguistic and cultural validation of the ROMP questionnaire, this instrument was translated and retranslated Relliability was verified through the analysis of internal consistency and reproducibility of the intra-examiner retest. The final version was used to assess 27 participants with Parkinson's disease (PD). Results: the final version of the PADAF consisted of 32 items that evaluated respiration, phonation, resonance, articulation, and prosody. It was shown to be valid, with a content validity index (CVI) well above that established elsewhere in reports, and in perfect concordance with the reliability verified between examiners. As for the ROMP questionnaire, the internal consistency was 0.99 for the total score and between 0.96 to 0.99 for the 3 domains. The intraclass correlation coefficients for reproducibility were 0.99 for the total score and between 0.93 to 0.99 for the subscales. The ROMP questionnaire and its subscales were significantly correlated with the Likert-type scale, as well as with UPDRS II and III. Conclusion: The PADAF for PD individuals was developed and had its content validated with perfect reliability of the instrument. Similarly, the linguistic and cultural equivalent of the ROMP questionnaire in Brazilian Portuguese was undertaken with excellent reliability and validity. Both instruments proved to be useful and applicable to this population.
