Abstract:
Die vorliegende Diplomarbeit ordnet sich in den Forschungskontext linguistischer gesprächsanalytischer Untersuchungen zu medizinischer Kommunikation ein. Die leitende Forschungsfrage widmet sich einem bisher kaum untersuchten Bereich der ÄrztInnen- PatientInnen- Kommunikation in hausärztlichen Ordinationen. Mit Hilfe gesprächsanalytischer Forschungsinstrumente sollen Erkenntnisse bezüglich der Unterschiede in der Kommunikation bei Wahl- und KassenärztInnen gewonnen werden. Das Datenmaterial stammt aus einem Forschungsprojekt von Florian Menz und Luzia Plansky (2010), in dessen Rahmen von insgesamt etwa 1000 aufgezeichneten Gesprächen 193 nach HIAT- Konventionen transkribiert wurden. Hiervon wurden insgesamt neun authentische Gesprächen bei verschiedenen niedergelassenen ÄrztInnen der Allgemeinmedizin sowie InternistInnen für die vorliegende Analyse ausgewählt. Audioversionen und Transkripte dieser Gespräche dienen schlussendlich der qualitativen Analyse, wobei hierbei ein Fokus auf Gesprächseröffnungen und Beendigungen, auf den Ablauf körperlicher Untersuchungen sowie auf ärztliche Reaktionen auf patientInneninitiierte Fragen und Thematisierung von Unsicherheiten gelegt wird. Die Analyse legt eindeutige Unterschiede in Gesprächsorganisation und -ablauf offen. Vorwiegend zeigen sich diese Differenzen in Form der ausführlicheren Informationsweitergabe, einer verhältnismäßig ausgeglichenen Redeverteilung sowie der Ermunterung zu aktiver Teilnahme von PatientInnen am Gespräch bei WahlärztInnen. Zusätzlich konnte aber festgestellt werden, dass die Vertragsart nicht zwangsläufig über den Grad an Effizienz der ÄrztInnen- PatientInnen- Kommunikation entscheidet, sondern vielfach auch mit begrenzten Zeitressourcen der KassenärztInnen produktive und zielführende Kommunikation stattfinden kann, sofern diesbezüglich genügend Initiativen von den jeweiligen ÄrztInnen erfolgen. ; This diploma thesis picks up on a sociolinguistic analysis of doctor-patient-communication in primary care encounters. It focuses on variations in doctor-patient-interaction which might be caused by different types of health insurance. Specifically there are two types of health insurance: one is provided by the state and often considered inferior in comparison to health care provided either by individuals or private insurance companies. This analysis aims to determine to what extend the existence of a “two-tier health care system” can be proven linguistically. Several hypotheses concerning certain presumptions about differences in the verbal behaviour of doctors are introduced. Using conversation-analytic methods, nine different conversations between doctors and patients in primary care encounters had been analysed. The selected data originates from a former project, conducted by Menz and Plansky (2010), which contains about 193 conversations between doctors and patients in primary care encounters. The material was collected in different locations within Austria and does not only contain audio recordings, but had also been transferred into a written form. Specializing in the analysis of certain aspects such as the organisation of opening and closing sequences, talk during physical examination as well as doctors´ reactions to questions of patients, it had been possible to draw a number of conclusions. Firstly the results show that there are indeed qualitative variations between doctor-patient-interactions caused by different types of health care insurances. Typically those patients who are treated as “private patients” are integrated actively during the whole encounter. Doctors provide more possibilities to ask questions and generally give more information throughout the interaction, especially during physical examinations. As a consequence, those patients have a better chance to be empowered in their position, which leads to a higher rate of compliance among this group of patients. Effective communication meets the demands of physicians and patients and their needs for clear and sufficient information. Furthermore it is compatible with constricted time ranges in those situations. The results show that while it might be easier for private patients to efficiently communicate with physicians during their consultations, successful interaction is not limited to this group. Empowering patients is therefore not confined to those doctors who have more time to spend with patients, but can be realised in temporally restricted encounters. Eventually it mostly depends on the doctors´ willingness and competence to strengthen patients´ status and advocate better opportunities to actively participate during their consultations.
