Bronchiectasis in Alaska Native children and Indigenous children from Central Australia
Abstract: Bronchiectasis in childhood has largely disappeared in developed countries except in association with cystic fibrosis and compromised pulmonary host defense. However, it remains an important cause of morbidity in developing countries and among Indigenous populations such as Alaska Native (AN), Australian Aboriginal and Torres Strait Islanders (AATSI) and Maori and Pacific Islanders in New Zealand. To compare characteristics of two groups of Indigenous children diagnosed with bronchiectasis we reviewed medical charts of 33 patients from Alaska and 65 from Central Australia. Two-thirds had their diagnosis based on HRCT, the remainder had clinical diagnosis with chest x-ray. Median age at diagnosis was 4.8 years for AN (range 1–15 years) and 5.3 years AATSI (range 8 months to 15 years). Underweight (weight for age z-score ≤−2), wasting (weight for height z-score ≤−2) and prematurity were more common among AATSI children (25% vs. 3% p < 0.001, 13% vs. 0% p < 0.001 and 28% vs. 12% p = 0.08 respectively). 92% of AATSI and 64% of AN children had at least one pneumonia hospitalization before diagnosis, and overall, AATSI children had more episodes (median 3.0, range 0–17 vs. 1.0, range 0–10, p < 0.001 respectively). However, more AN children (86%) had their first hospitalization in the first 2 years of life when compared to AATSI children (23%, p < 0.001). In addition, more AN have been diagnosed (66%) and hospitalized with asthma (46%) compared with AATSI (23% and 5% respectively, both p < 0.001). Conclusion The differences in asthma and pneumonia patterns between the 2 groups could reflect differences in the presentations of disease. This should be further explored to improve our understanding of bronchiectasis in Indigenous populations.
