Abstract:
The study of cognitive changes following Parkinson's disease was carried out within the framework model in order to move from the simple description of the disorders to the explanation of the deficits in processual terms. The Baddeley working memory model and the Norman and Shallice's Supervisory Attention System model were used. Examination of the different components of working memory by adapted neuropsychological tasks shows that the deficits were not manifest at the level of non-specific factors such as phasic alertness or processing speed. It appears neither at the level of the temporary storage capacity nor of the functionality of the slave systems (Phonological and Articulatory Loop and Visuo-Spatial Sketchpad). However, Parkinson's disease patients were deficient in the functionality of the central administrator in both the management of attentional resources and the executive control of actions. The deficits were salient in action planning, inhibition of dominant responses, and cognitive flexibility. The theoretical models used have proven to be a useful metaphor for accounting for the deficits observed. However, these models still lack specificity since the central administrator deficit is also observed in other pathological populations. Further work to question models is essential. Examining heterogeneity in the expression of disorders would be able to help specify the nature of central administrator deficits by identifying different patterns of working memory dysfunction. ; L'étude des changements cognitifs consécutifs à la maladie de Parkinson est effectuée dans le cadre d'un modèle théorique de référence afin de passer de la simple description des troubles à l'explication des déficits en termes de processus. Le modèle de mémoire de travail Baddeley et son corollaire celui du Système Attentionnel Superviseur de Norman et Shallice sont utilisés. L'examen des différentes composantes de la mémoire de travail par des tâches neuropsychologiques adaptées montre que le déficit ne se manifeste pas au niveau des facteurs non spécifiques comme l'alerte phasique ou la vitesse de traitement. Il ne se manifeste pas non plus au niveau de la capacité de stockage temporaire ou de la fonctionnalité des systèmes esclaves (Boucle Phonologique et Articulatoire et Registre Visuo-Spatial). En revanche, les parkinsoniens sont déficients dans la fonctionnalité de l'administrateur central dans sa fonction de gestion de ressources attentionnelles et dans des fonctions de contrôle exécutif de l'action aussi bien au niveau de la planification de l'action qu'au niveau de l'inhibition des réponses dominantes et de la flexibilité mentale réactive et spontanée). Les modèles théoriques utilisés se sont avérés représenter une métaphore utile pour rendre compte des déficits observés. Cependant, ces modèles manquent encore de spécificité puisque le déficit de l'administrateur central est observé aussi dans d'autres populations pathologiques. D'autres travaux de remise en question des modèles sont indispensables. L'examen de l'hétérogénéité dans l'expression des troubles serait en mesure d'aider à spécifier la nature des déficits de l'administrateur central en dégageant différents patterns de dysfonctionnement de la mémoire de travail.
