學前發展性語言障礙兒童表達性語言能力、語速和不流暢關係之研究 ; A Study on Comparing the Speech Rate, Disfluency and Expressive Abilities between Preschool Children with Developmental Language Disorder and Typical Development
Abstract: 本研究之目的一在探討學前發展性語言障礙(DLD)兒童與一般兒童在看圖說故事之語速、言語不流暢和表達性語言能力的差異情形;研究目的二在探討全體兒童在看圖說故事的語速、言語不流暢和表達性語言能力彼此之間是否有顯著相關。研究目的三則是在探討全體兒童在語言樣本分析的口語能力相關變項對於口語表達測驗是否具有顯著預測力。研究者召募4-6歲之30位學前DLD兒童和30位一般兒童,共計60位為本研究對象,蒐集兒童看圖說故事的語言樣本。先將語言樣本轉錄為逐字稿,整理各變項資料,言語不流暢包括:口吃式不流暢、其他不流暢和整體不流暢,表達性語言能力包括:語意向度的總音節數、總詞數、相異詞數和修正後相異詞彙比率;語法向度的MLU-c、MLU-w、MLU5-c和MLU5-w,以及標準化口語表達能力測驗,再以獨立樣本t檢定來比較兩組兒童的差異情形,以皮爾森積差相關探討兩組兒童語速、言語不流暢和表達性語言能力之相關,最後,將具有顯著相關之變項對口語表達測驗表現進行逐步回歸分析。 本研究結果如下:一、相較於一般兒童,DLD兒童的語速顯著為慢;口吃式不流暢、其他不流暢和整體不流暢出現頻率顯著為多;表達性語言能力(語意向度、語法向度和標準化測驗分數)顯著落後,且DLD兒童的口吃式不流暢已接近口吃診斷標準。二、全體學前兒童之語速和言語不流暢三項指標有顯著負相關;語速和表達性語言能力(語意向度四項指標語、語法向度四項指標和口語表達測驗分數)有顯著正相關;不流暢(口吃式不流暢、其他不流暢和整體不流暢)和語意向度四項指標、語法向度四項指標、口語表達測驗分數有顯著負相關;表達性語言能力指標(語意向度和語法向度)的各四項指標和口語表達測驗有顯著正相關。三、平均語句詞數、修正後相異詞彙比率、托尼非語文智力測驗分數和SLD可以聯合預測90.2%的全體兒童口語表達能力測驗表現,其中以平均語句詞數預測力最大佔86%,其餘三者解釋力小,可以忽略。 歸納本研究發現,包括:1.語速、不流暢(口吃式不流暢、其他不流暢和整體不流暢)、表達性語言能力(語意向度、語法向度和表達性語言測驗表現)皆可用以區別學前DLD與一般兒童。2.語速和口語表達能力為正相關,但與不流暢為負相關。3. 不流暢和口語表達能力為負相關。4.表達性語言能力之語言樣本分析指標和標準化口語表達測驗表現為正相關。5.平均語句詞數對標準化口語表達測驗表現預測力最高,有86%。最後討論本研究結果並提出相關建議以供未來研究參考。 ; The purposes of this research is to explore while telling a picture story, (I) whether differences existed between the preschool DLD and non-DLD children in the speech rate, disfluencies (stuttering-like disfluency, other disfluency and total disfluency) (SLD, OD, and TD), the expressive abilities (semantic domain: number of total syllables, number of total words, number of different words and corrected total token ratio; syntactic domain: Mean length of utterance-syllable, Mean length of utterance-word, Mean length of the 5 longest utterance-syllable, Mean length of the 5 longest utterance-word )(NTS, NTW, NDW, CTTR; MLU-c, MLU-w, MLU5-c, MLU5-w), and the standardized expressive performance; (II) For all the children, whether the measures in speech rate, disfluency and expressive abilities are significantly related to each other; and (III) For all the children, whether the measures in speech rate, disfluency, and expressive abilities have a significant predictive power for the performance on the standardized expressive instrument, respectively. The 30 pairs aged 4-6 who were with DLD and without DLD in preschool were recruited and all of their speech samples were collected while telling a picture story. Then, the speech samples were transcribed into verbatim and all the above measures were calculated. There were t tests, Pearson product-moment correlations and a stepwise regression analysis which were used to answer the three research questions. The results of this study are as follows: 1. Compared with non-DLD children, the DLD children had significantly slower speech rates; higher SLD, OD and TD; the poorer expressive abilities in the respective semantic domain, syntactic domain and standardized instrument. The DLD children are also very close to the diagnostic criteria for having stuttering. 2. For all the preschool children, there were significant negative correlations between the speech rate and disfluencies (SLD, OD and TD), respectively; significant positive correlations between the speech rate and all the measures in the expressive abilities; significant negative correlations between disfluencies (SLD, OD, and TD) and all the measures in the expressive abilities, respectively; significant positive correlations between all the measures in the expressive abilities and the performance on the standardized expressive instrument. 3. For all the preschool children, MLU-w, CTTR , TONI-4 and SLD can jointly predict 90.2% of the performance on the standardized expressive instrument. Among the three measures, MLU-w has the largest predictive power of 86%, while the other three have small powers and can be ignored. In summary, 1. all the measures in the speech rate, disfluency, expressive abilities can be used to distinguish the DLD from non-DLD children in the preschool. A child may have both DLD and stuttering. 2. The speech rate and all expressive abilities are positively correlated, but negatively correlated with disfluencies. 3. The disfluencies are negatively correlated with all the expressive abilities. 4. Both the semantic domain and syntactic domain of the expressive abilities and the performance on the standardized expressive instrument are positively correlated, respectively. 5. MLU-w has the highest predictive power on the performance on the standardized expressive instrument, with 86%. Finally, some discussions regarding the results obtained and several suggestions for the future research were proposed.
