Abstract:
INTRODUÇÃO: a experiência auditiva com música é um desejo expresso por muitos usuários de Implante Coclear (IC) e pode ser um importante indicador dos benefícios desta tecnologia. Uma experiência auditiva musical mais aprimorada é capaz de contribuir para uma melhor compreensão auditiva no complexo ambiente acústico vivenciado pelos usuários de IC, bem como representar um impacto positivo em sua integração social e qualidade de vida. OBJETIVO: avaliar a relação existente entre percepção e apreciação musical, reconhecimento de fala e qualidade de vida, em adultos usuários de Implante Coclear. METODOLOGIA: estudo observacional e transversal em 20 adultos usuários de IC, de ambos os sexos, com idade variando entre 18 e 60 anos. A avaliação da percepção da fala foi realizada por meio da aplicação de lista de sentenças e de palavras monossílabas, apresentadas a 65 dBNA, em sistema de campo sonoro e em cabina acústica. Para a avaliação da percepção musical, foram utilizados os subtestes de discriminação de pitch e de identificação de timbre existentes na Avaliação Clínica da Percepção de Música da Universidade de Washington - CAMP. Para a avaliação da apreciação musical foi utilizado o Questionário de música de Munich MUMU e a avaliação da qualidade de vida foi obtida a partir da aplicação do Questionário Nijmengen de Implantes Cocleares NCIQ-P. RESULTADOS: os resultados de percepção musical demonstraram desempenho médio para discriminação de pitch de 4,79 semitons. Para a identificação de timbre, piano e violão foram os instrumentos identificados com maior frequência, com desempenho médio de 25,83%. Não foi encontrada uma relação estatisticamente significante entre a frequência em ouvir música e a discriminação de pitch. Os sujeitos de pesquisa que referiram uma importância média da música em suas vidas apresentaram melhor desempenho na tarefa de identificação de timbre. O desempenho de reconhecimento de fala, bem como as variáveis idade, tempo de privação sensorial e tempo de uso do IC não estiveram estatisticamente associadas à discriminação de pitch. Quanto maior foi a idade dos sujeitos, pior foi o desempenho para a identificação de timbre. A pontuação média obtida a partir do questionário de qualidade de vida foi de 53,3%, sendo o domínio físico o mais bem pontuado entre os participantes da pesquisa. Uma associação estatisticamente significante foi encontrada entre a pontuação obtida para o domínio físico e a identificação de timbre. CONCLUSÃO: a utilização de uma avaliação clínica combinada, capaz de refletir de maneira fidedigna a natureza dinâmica e multifacetária da experiência musical na vida real, pode ser capaz de impactar positivamente a experiência musical e a qualidade de vida da população usuária de IC ; INTRODUCTION: many users of cochlear implants (CI) express a desire to listen to music and this can be an important indicator of the benefits this technology offers. An improved auditory experience of music contributes to a better auditory understanding in the acoustic complex experienced by CI users, and it has a positive impact on social integration and quality of life for CI users. OBJECTIVE: to evaluate the relationship between musical perception and appreciation, speech recognition, and quality of life in adult cochlear implant users. METHODOLOGY: observational and cross-sectional study of 20 adult users of CI, of both sexes aged from 18 to 60 using. Speech perception was evaluated by applying a list of sentences and monosyllabic words, at 65 dBNA, in sound field system in acoustic booth. Musical perception was evaluated by subtests of pitch discrimination and timber identification in the University of Washingtons Clinical Evaluation of Music Perception (CAMP). Musical appreciation was evaluated by the Munich Music Questionnaire (MUMU), and quality of life was assessed by application of the Nijmegen Cochlear Implant Questionnaire (NCIQ-P). RESULTS: the results of musical perception demonstrated an average performance for pitch discrimination of 4.79 semitones. For the identification of timbre, the piano and guitar were the most frequently identified instruments, with an average performance of 25.83%. No statistically significant relationship between frequency of listening to music and pitch discrimination was found. The research subjects who reported an average importance of music in their lives presented better performance in the task of timbre identification. Speech recognition performance, as well as the variables of age, time of sensory deprivation, and time of CI use, were not statistically associated with pitch discrimination. The older the subjects, the worse was the performance in identification of timbre. The average score obtained from the quality-of-life questionnaire was 53.3%, with physical domain being the highest score among the participants. No statistically significant association was found between the score obtained for physical domain and the identification of timbre. CONCLUSION: the use of a combined clinical evaluation, capable of reliably reflecting the dynamic and multifaceted nature of the musical experience in real life, may positively impact musical experience and quality of life for users of cochlear implants
