Abstract:
National audience ; But : Les séquelles d’atteinte du nerf laryngé supérieur (NLS) après chirurgie thyroïdienne ont été évaluées par des paramètres acoustiques et par leurs retentissements sur la prosodie, en corrélant les résultats aux ressentis. Méthodes : 30 femmes avec suspicion d’atteinte du NLS ont été comparées à 30 autres sans dommage post-chirurgical, et à 30 femmes témoins appariées en âge. La fréquence fondamentale (F0) maximale (F0max), minimale (F0min), leur différence (étendue vocale : diff F0max-F0min), la durée des groupes rythmiques ainsi que l’intensité relative ont été mesurées sur des épreuves vocales et prosodiques. Résultats : Un abaissement des F0 moyennes et maximales avec une réduction de l’étendue vocale maximale d’une tierce sur un texte lu était spécifique à l’atteinte du NLS. (F0moy : F(1,58)=20,56; p<0,0001, F0max : F(1,58)=40,51;p<0,0001 et diff F0max-F0min : F(1,58)=38,06; p<0,0001). L’expression prosodique d’une question et d’une exclamation était impactée : différences entre les groupes rythmiques, durée des groupes allongée et impossibilité d’attaque en registre haut. Toutefois, la seule incidence à 6 mois de la thyroïdectomie n’était pas sans conséquence avec notamment un défaut de contrôle de l’intensité (shimmer augmenté : F(1,58)=4,25; p=0,0436) . Cependant, 14% des patients avec atteinte du NLS rapportaient un handicap vocal modéré à sévère contre 4% pour les autres patients (F(1,50)=7,69; p=0,0247). Enfin, ce ressenti était révélateur des altérations acoustiques (F0max (texte) et questionnaire spécifique : F(1,49)=23,41; p<0,0001).Conclusion : Le diagnostic d’atteinte du NLS peut être facilité par une évaluation de l’étendue vocale (texte) et un test de paires minimales intonatives, corrélés à une auto-évaluation du handicap vocal. Ces analyses sont facilement reproductibles en clinique notamment si un electromyogramme est indisponible.
