Quelques autoquestionnaires sont disponibles pour évaluer les limitations d’activités et les restrictions de participation, comme le PHI [3]. D’autres outils permettent une évaluation de la dynamique communicationnelle [4, 5], mais ne sont pas validés en cancérologie. Ces différents outils aboutissent à des scores globaux additifs ou moyennés, peu représentatifs des limitations de communication réellement perçues par les patients. L’objectif est ici de construire un score holistique permettant de représenter au mieux l’impact fonctionnel sur la communication des troubles de la parole chez les patients traités pour un cancer de la cavité buccale ou de l’oropharynx. Différents questionnaires et auto-questionnaires concernant la dynamique communicationnelle, l’impact pluridimensionnel du trouble de la parole, le handicap de parole perçu ou la qualité de vie relative à la parole [3-7] ont été proposés à 25 patients (âge médian 67 ans, EIQ 12 ; 15 hommes, 10 femmes ; cavité buccale : 14, oropharynx : 10, deux localisations : 1). Deux cent dix items issus des questionnaires ont été recueillis comme indicateurs du score holistique de communication (variable latente). Une méthodologie de réduction de dimensionnalité a été ensuite appliquée pour construire un score quantitatif. La validité d’apparence a permis d’éliminer les items non pertinents : un comité d’experts de différentes disciplines concernées par la communication (sciences du langage, médecine, informatique et traitement du signal, orthophonie) a été sondé pour définir les critères d’inclusion des items dans notre score holistique. Puis, une étude de la validité de construit, au moyen d’une matrice de corrélation inter-items, a permis d’éliminer les items redondants et insuffisamment variables pour obtenir un score permettant la discrimination inter-individuelle sur l’impact communicationnel. Enfin une analyse factorielle en principal-component factor [8] a été appliquée, suivie d’une prédiction des scores post-analyse et un changement d’échelle. La validation croisée à cinq blocs montre une stabilité de cet indicateur holistique. La fiabilité temporelle de ce score reste encore à analyser, mais cette étude a permis d’élaborer un score global permettant une mesure de l’impact du trouble de parole après traitement d’un cancer oral ou oro-pharyngé sur la communication. Il permet de combler l’absence de ce type d’outils en cancérologie ORL, et peut autoriser une meilleure appréhension de la qualité de vie relative à la parole.
