Hearing Loss and Sound Amplification: Effects on the Central Auditory System.

Abstract: Auditory deprivation is known to cause functional and structural abnormalities in the central auditory pathways. The symptoms of hearing loss include poor speech discrimination in noise, distortions of loudness perception, and the emergence of phantom sounds (tinnitus) and are hypothesized to occur from the brain's reaction to hearing loss. Intervention with hearing aids that amplify acoustic signals have been developed to restore hearing. However, satisfaction with such devices has been lacking. There are two sides to this question: why don't hearing aids satisfy the users and what are the effects of hearing aids on the brain? Our experiments were designed to reduce the severity of hearing loss. We evaluated the relationship between the severity of hearing loss, the intervalbetween hearing loss and intervention, and the impact of restored hearing on the brain. We hypothesized that timely intervention thatamplifies acoustic signals would delay further progression of hearing loss by maintaining auditory activity and structure.To that end, we provided eight weeks of acoustic stimulation that was tailored to compensate those frequencies of hearing loss in mouse models of progressive hearing loss. We used DBA/2 and C57BI/6 mice, models of early-onset and adult-onset hearing loss, respectively. We evaluated the weekly progression of hearing status by examining parameters of the evoked auditory brainstem responses in mice with or without sound amplification. At the end of stimulation, mice received anterograde neurotracer (neurobiotin) injections into the auditory nerve, followed by tissue processing for immunohistochemistry and confocal microscopy. Analysis compared data from untreated control mice to treated experimental mice and was conducted by a "blinded" observer.Early treated subjects showed delayed progression of hearing loss as evidenced by smaller threshold shifts and larger amplitudes of response. There was a reduced loss of inhibitory glycinergic input but the most striking result was that treatment produced more robust auditory nerve terminals in the cochlear nucleus compared to controls. This result implies that auditory stimulation is a good thing. However, the modest impact of amplified sound to the genetic models of hearing loss emphasized that genes are a dominant force.
