Síntomas cognitivos en la COVID-19 persistente: un análisis neuropsicológico

Abstract:
Recientes evidencias han puesto de manifiesto como el nuevo coronavirus, que causa la enfermedad COVID-19, puede afectar al sistema nervioso central y provocar alteraciones cognitivas de naturaleza y gravedad variable en las personas que lo padecen. Más específicamente, en los últimos meses, se ha definido el subtipo de COVID-19 persistente para referirse a aquellas personas que han superado la infección aguda de la enfermedad, pero manifiestan síntomas variados e inespecíficos que persisten meses después de la recuperación. El objetivo de este trabajo fue estudiar los síntomas cognitivos de personas con COVID-19 persistente, utilizando diferentes tests neuropsicológicos. Participaron 33 adultos españoles, de entre 35 y 63 años, que habían padecido COVID-19 y mostraban síntomas que persistían en el momento de la evaluación. Las puntuaciones obtenidas en los diferentes tests neuropsicológicos, fueron comparadas con los baremos del proyecto NEURONORMA y en su defecto, con los baremos de los tests específicos. Las principales afectaciones se encontraron a nivel del lenguaje, la velocidad de procesamiento y las funciones ejecutivas, especialmente en la capacidad de inhibición y de razonamiento. Se encontró una relación inversa entre la gravedad de la enfermedad y las puntuaciones en la prueba de aprendizaje verbal, así como diferencias significativas en el rendimiento de pruebas entre pacientes con patologías previas como hipotiroidismo o HTA. Más allá de los déficits cognitivos asociados a la fase aguda del COVID-19, estos resultados ponen de manifiesto la alta prevalencia de alteraciones cognitivas. ; The new evidence has shown how the new coronavirus, that causes COVID-19 disease, can affect the central nervous system and cause cognitive impairments with different severity and characteristics in people who suffer from it. More specifically, in recent months the concept of Long-COVID has been introduced, referring to those persons who overcome the acute infection of the disease, but still show unspecific and mixed symptoms that persist for months after recovery. The aim of this work was to study the cognitive symptoms of people with Long-COVID, by using several neuropsychological tests. The participants were 33 Spanish adults, aged 35 to 63 years, who had suffered from COVID-19 and showed symptoms that persisted at the time of the assessment. The scores obtained in the evaluation were compared with those of the NEURONORMA project and, if applicable, with the specific test scales. The main impairments were found at language, speed processing and executive functions, especially in the inhibition and reasoning ability. In addition, an inverse correlation between disease severity and the scores on the verbal learning test was found. Significant differences showed in some tests when patients with previous pathologies such as hypothyroidism or HTA were compared. Besides the cognitive deficits associated with the acute phase of COVID-19, these results highlight the high prevalence of persistent cognitive symptoms in patients who have recovered from the clinical phase of infection. ; Recents evidències han posat de manifest com el nou coronavirus, que causa la malaltia COVID-19, pot afectar el sistema nerviós central i provocar alteracions cognitives de naturalesa i gravetat variable en les persones que el pateixen. Més específicament, en els últims mesos, s'ha definit el subtipus de COVID-19 persistent per a referir-se a aquelles persones que han superat la infecció aguda de la malaltia, però manifesten símptomes variats i inespecífics que persisteixen mesos després de la recuperació. L'objectiu d'aquest treball va ser estudiar els símptomes cognitius de persones amb COVID-19 persistent, utilitzant diferents tests neuropsicològics. Van participar 33 adults espanyols, d'entre 35 i 63 anys, que havien patit COVID-19 i mostraven símptomes que persistien en el moment de l'avaluació. Les puntuacions obtingudes en els diferents tests neuropsicològics, van ser comparades amb els barems del projecte NEURONORMA i en defecte d'això, amb els barems dels tests específics. Les principals afectacions es van trobar a nivell del llenguatge, la velocitat de processament i les funcions executives, especialment en la capacitat d'inhibició i de raonament. Es va trobar una relació inversa entre la gravetat de la malaltia i les puntuacions en la prova d'aprenentatge verbal, així com diferències significatives en el rendiment de proves entre pacients amb patologies prèvies com a hipotiroïdisme o HTA. Més enllà dels dèficits cognitius associats a la fase aguda del COVID-19, aquests resultats posen de manifest l'alta prevalença d'alteracions cognitives.
