Material educativo sobre o estigma à saúde mental entre profissionais da saúde no atendimento a pacientes de difícil manejo de um hospital geral
Abstract: Introdução: A literatura mostra que o estigma por parte dos profissionais da saúde de unidades clínicas aos pacientes com transtornos psiquiátricos graves pode prejudicar a qualidade no atendimento prestado a eles. Porém, existem poucos dados referentes ao estigma relacionado aos considerados “pacientes difíceis”, caracterizados por comportamentos excessivamente opositores ou demandantes com a equipe, muitas vezes decorrentes de algum transtorno psiquiátrico não diagnosticado ou não adequadamente tratado. Objetivo: elaborar um material educativo a partir dos resultados de uma pesquisa exploratória sobre as características do estigma relacionado aos chamados “pacientes difíceis” entre profissionais da saúde de um hospital geral da rede privada. Material e Método: foi realizada uma revisão narrativa de escalas que avaliavam o estigma à doença mental, elaborado um questionário exploratório adaptado ao estigma de profissionais de saúde aos pacientes de difícil manejo, aplicado o questionário no formato on-line entre profissionais de saúde de um hospital privado das áreas da enfermagem, farmácia, fisioterapia, fonoaudiologia, medicina, nutrição, psicologia e serviço social e elaborado um material educativo com base nos resultados da pesquisa. Resultados: o instrumento resultou num questionário exploratório auto-aplicável com 29 itens distribuídos em três domínios principais (conhecimento, preconcepções e comportamentos), 144 profissionais da saúde responderam ao instrumento e apresentaram escores médios para o estigma ao paciente difícil, não apresentaram diferenças significativas quanto aos escores em relação às diferentes categorias profissionais, idade, tempos de experiência profissional ou institucional considerando os escores dos três domínios explorados. A partir dos resultados foi criado uma série de 4 episódios em “podcasts” sobre o estigma aos pacientes difíceis e estratégias de manejo destinada aos profissionais de saúde desta instituição. Conclusão: este trabalho propõe um instrumento de avaliação do estigma de profissionais de saúde a um perfil de pacientes comum nos cenários de atendimento e resulta na elaboração de uma série de “podcasts” com conteúdos educativos de apoio a este público sobre o tema. ; Introduction: The literature presents the provider’s stigma among patients with serious mental disorders in clinical units as a mediator for health care quality damages of those patients. However, few data about the stigma associated to difficults patients care has been published. Difficult patients are featured for being excessively opponents, extremely demanders and quite often presenting undiagnosed or untreated mental health disorders. Objective: to elaborate an educational content from the results of an exploratory research about mental health provider’s stigma characteristcs in a private general hospital. Material and Method: we conducted narrative review about mental health stigma scales, elaborated an adapted exploratory questionnaire of providers’ mental health stigma to the difficult patients construct, applied this questionnaire to health care professionals of medicine, nursing, nutrition, pharmacy, physiotherapy, psychology, social service and speech therapy areas and produced an educational content from the results of this research. Results: the instrument resulted in a 29 itens self-appliable exploratory questionnaire distributed in three domains (knowledge, attitudes and behaviors), 144 health professionals answered the instrument and presented a medium level of stigma for the difficult patients. We didn’t find differences in scores according to health care professions, the research didn’t present correlations between age, professional experience or institutional times with the stigmas scores and didn’t show differences between the three analyzed stigma domains. From these results, we elaborate a four episodes podcasts serie about the difficult patients providers’s stigma and management strategies destinated to those health care professionals. Conclusion: this study purposed an new mental health providers’s stigma instrument adapted to the difficult patient construct quite common in the clinical assistance settings and resulted in a podcasts’ serie as an educational content support to those institutional health care professionals.
