Abstract:
Trabalho final do 5º ano com vista à atribuição do grau de mestre no âmbito do ciclo de estudos de Mestrado Integrado em Medicina Dentária apresentado à Faculdade de Medicina da Universidade de Coimbra. ; Introdução: A Paralisia Cerebral é descrita como um conjunto de perturbações do desenvolvimento motor e postural com limitações a vários níveis, atribuídas a distúrbios não progressivos que ocorrem no cérebro fetal ou infantil em desenvolvimento. Alguns autores referem que as crianças com paralisia cerebral podem apresentar maior susceptibilidade a diferentes patologias orais, inerentes a condicionantes na relação direta ou indireta da doença, nomeadamente alterações anatómicas, fisiológicas, terapêuticas e comportamentais. Objetivos: O principal objetivo deste trabalho residiu na recolha de informação sobre o grau de conhecimentos relativos à saúde oral em crianças com paralisia cerebral frequentadoras da Associação de Paralisia Cerebral de Coimbra, de forma a contribuir para a optimização de cuidados preventivos específicos. Complementarmente, foi desenvolvida uma revisão bibliográfica acerca das patologias orais mais prevalentes nestas crianças, bem como da influência dos pais/tutores nos cuidados de saúde oral das mesmas. Metodologia: Realizaram-se 41 inquéritos (baseados no modelo “Child Oral Health Impact Profile”) aos pais/tutores de crianças dos 2 aos 17 anos de idade com diagnóstico de paralisia cerebral frequentadores em permanência do jardim de infância e escola EB1APCC Coimbra Sul, bem como em regime ambulatório. Os resultados obtidos, depois de inseridos em Microsoft Excel®, foram sujeitos a análise estatística descritiva através do programa IBM SPSS v.19. A pesquisa bibliográfica foi realizada na PubMed/MEDLINE, complementada com consulta manual, utilizando as combinações de palavras-chave “cerebral palsy and oral health” AND “cerebral palsy and quality of life” AND “cerebral palsy and pediatric dentistry” AND “cerebral palsy and oral manifestations” AND “cerebral palsy and disability” AND “cerebral palsy and access to dental care” AND “cerebral palsy and function impairment” AND “motor deficiency and oral hygiene”, das quais são termos Mesh “dental care”, “cerebral palsy / epidemiology”, “cerebral palsy / classification”, “cerebral palsy / physiopathology”, “quality of life” e “oral health”, contemplando-se os seguintes critérios de inclusão: publicações de 2008-2013, escritos em inglês e português, com resumo disponível e após avaliação do respetivo conteúdo científico. Resultados: Com maior relevância destaca-se que a totalidade dos pais/tutores consideraram importante a higiene oral das crianças; 82.9% referiram supervisionar a escovagem, em 53.7% bi-diária. Cerca de 70% desconhece a existência de meios adaptados às necessidades de higiene oral, não usando qualquer meio complementar; 73.2% não os procuraram, no entanto, 90.6% se tivessem acesso, usá-los-iam. Paralelamente, 65.9% nunca teve experiência prévia de odontalgia e 88% referiu a inexistência de hemorragia gengival associada à escovagem. Na revisão bibliográfica foram inicialmente obtidas 452 referências, selecionando-se 22 após eliminação de duplicados e aplicação dos critérios de inclusão; a estas acresceram 15 referências cruzadas destacando-se, em termos de resultados, a menção a uma ocorrência acrescida de manifestações orais que possam advir da paralisia cerebral. Conclusão: É prioritária a instrução e orientação dos pais/tutores para uma melhoria dos cuidados de saúde oral no quotidiano destas crianças, com implementação de programas de promoção da saúde oral e divulgação da informação relativa aos cuidados em ambulatório. A atuação concertada de uma equipa multidisciplinar é essencial para a melhoria da sua qualidade de vida, impelindo a que sejam ultrapassadas todas as barreiras linguísticas, culturais e financeiras que possam impedir ou condicionar o desejável acesso aos cuidados devidos. Introduction: Cerebral palsy describes a group of permanent disorders of the development of movement and posture, causing activity limitation, attributed to non-progressive disturbances that occurred in the developing fetal or infant brain. Some authors have reported that children with cerebral palsy may have greater susceptibility to different oral pathologies, constraints inherent in direct or indirect relation of the disease, including anatomical, physiological, therapeutic and behavioural changes. Objective: The main objective of this work resided in the collection of information about the degree of knowledge related to oral health in children with cerebral palsy attending the Cerebral Palsy Association of Coimbra in order to contribute for the optimization of specific preventive care. In addition, it was developed a literature review about the most prevalent oral diseases in these children, as well as the parents / tutors influence on their oral health care. Methods: There were carried out 41 inquiries (based on the model "Child Oral Health Impact Profile") to parents / tutors of children aged 2 to 17 years old with cerebral palsy goers permanently in the kindergarten and school EB1APCC Coimbra Sul, as well as outpatient. The results obtained, after inserted into Microsoft Excel®, were subjected to descriptive statistics analysis using the program SPSS V.19. A literature search was performed in MEDLINE/PubMed, complemented with manual query, using combinations of keywords "cerebral palsy and oral health" AND "cerebral palsy and quality of life" AND "cerebral palsy and pediatric dentistry" AND "cerebral palsy and oral manifestations" AND "cerebral palsy and disability" AND "cerebral palsy and access to dental care" AND "function impairment and cerebral palsy" AND "motor deficiency and oral hygiene", which are Mesh Terms "dental care", “cerebral palsy / epidemiology", "cerebral palsy / classification", "cerebral palsy / physiopathology", "quality of life" and “oral health", covering the following inclusion criteria: publications 2008-2013, written in English and Portuguese, with abstracts available and after evaluation of the relevant scientific content. Results: With greater relevance stands out that all parents / tutors considered important children oral hygiene, 82.9% reported supervise brushing, in 53.7% bi-daily. About 70% were unaware of the existence of resources adapted to the needs of oral hygiene, not using any additional means, 73.2% didn’t search them however, 90.6% if they had access, they would use it. At the same time, 65.9% never had prior experience of toothache and 88% referred the absence of gingival bleeding associated with brushing. In the literature review were initially obtained 452 references, after the elimination of duplicates and applying the inclusion criteria were selected 22; supplemented with 15 cross references, highlighting in outcomes, the mention of an increased occurrence of oral manifestations that may arise from cerebral palsy. Conclusions: The instruction and guidance of parents / tutors it’s a priority for the improvement of oral health care in daily life of these children, along with the implementation of programs to promote oral health and dissemination of information relating to outpatient care. The concerted action of a multidisciplinary team is essential for the improvement of their quality of life, urging that all barriers linguistic, cultural and financial that may prevent or restrict access to desirable due care are overcome.
