Desfecho alimentar pediátrico no momento da alta hospitalar : aspectos fonoaudiológicos
Abstract: Objetivo: Caracterizar os pacientes avaliados pela fonoaudiologia na internação pediátrica de um hospital terciário e seus desfechos alimentares. Método: Trata-se de um estudo quantitativo de delineamento transversal retrospectivo descritivo, realizado por análise de prontuários, com pacientes internados na unidade de internação pediátrica. Foram excluídos pacientes internados cujas consultorias não foram respondidas antes da alta, pacientes cuja avaliação não tenha sido completa, pacientes cuja participação em projetos tenha sido vetada pelos responsáveis e pacientes pediátricos internados em outras unidades do hospital. Para descrição das características utilizou-se a distribuição de frequências absolutas e relativas, mediana e faixa de distribuição. Para as associações entre as variáveis utilizou-se os testes qui-quadrado de Wald, exato de Fisher, McNemar, correção de continuidade e modelo de regressão Poisson com variação robusta, (p-valor<0,05). Resultados: Dos 386 pacientes da amostra, 56% eram do sexo masculino, 41,2% tiveram nascimento prematuro. A mediana de idade foi de 190 dias. Os grupos diagnósticos mais frequentes foram doenças do sistema respiratório (52,3%) e sintomas, sinais e achados clínicos e laboratoriais anormais, não classificados (37,8%). Com relação ao desfecho alimentar no momento da alta, 175 (45,3%) tiveram alta com via oral exclusiva. Houve associação estatisticamente significativa entre impressão diagnóstica de disfagia inicial e/ ou final com alguns dos grupos diagnósticos. Conclusão: Dentre os pacientes atendidos pelos fonoaudiólogos verificou-se maior frequência de doenças respiratórias e diagnóstico fonoaudiológico de disfagia orofaríngea. Na alta hospitalar, o desfecho alimentar com via oral exclusiva foi o mais frequente. ; Purpose: To describe the characteristics of the patients evaluated by speech therapist team in a tertiary hospital's pediatric unit and their feeding status outcome at discharge. Methods: This is a quantitative descriptive retrospective cross-sectional study. Data used for analysis were retrieved from electronic health records. Exclusion criteria: patients without a complete evaluation by the speech therapist, and patients hospitalized outside pediatrics unit. To verify the associations between variables, Wald Chi-Square Test, Fisher’s Exact Test, McNemar Test, Continuity Correction and Poisson regression model were used, with a significance level of 0,05. Results: A total of 386 patients were included in the study: 56% were male, 41,2% were born preterm. The median age was 190 days. The most common diagnosis were: respiratory system diseases (52,3%) and symptoms, signs and abnormal clinical and laboratory findings, not elsewhere classified (37,8%). Regarding the feeding status at hospital discharge, 175 (45,3%) were exclusively fed by mouth. Statistically significant association was observed between initial and final dysphagia diagnosis and some of medical groups of diagnoses. Conclusion: Respiratory diseases were the most common medical diagnosis among patients who received speech therapy intervention. Patients affected by endocrine, nutritional and metabolic diseases had a higher occurrence of food refusal. Most patients were discharged with an exclusive oral feeding.
